What does a tip suggestion line look like on a restaurant receipt?

Most POS systems print three tip suggestions below the total: '18%: $X.XX / 20%: $X.XX / 22%: $X.XX'. These are calculated on the pretax subtotal (not on the total after tax). Below those suggestions is a blank line for the customer to write in their chosen tip amount and a grand total line to fill in. On the receipt, auto-gratuity appears as a printed line item (e.g. 'Auto-Gratuity 18%: $16.45') with the tip already included in the total. The blank tip line is omitted. Auto-gratuity must be disclosed on the menu and the receipt must clearly label it as a service charge, not a voluntary tip.
