What do these medical receipt examples show?
Each example is a fully formatted medical receipt generated by our free tool. They show the provider name, NPI number, and address; patient name and date of service; CPT procedure codes with descriptions; ICD-10 diagnosis codes; the billed amount, insurance contractual adjustment, allowed amount, insurance payment, and patient responsibility (copay, deductible, or coinsurance): exactly what a medical provider's patient receipt or Explanation of Benefits (EOB) shows.
Can I create my own receipt like these examples?
Yes. Every example on this page was created using our free Medical Receipt Generator. Click 'Create Your Own' on any example to go directly to the generator with a similar template pre-loaded.
What is the difference between a medical receipt and an EOB?
A medical receipt (or patient receipt) is issued by the provider at the time of payment and shows the amount the patient actually paid. An Explanation of Benefits (EOB) is issued by the insurance company and explains how the claim was processed: including billed amount, allowed amount, insurance payment, and patient responsibility. Our medical receipt generator combines both into a single document showing the full payment breakdown, making it suitable for HSA/FSA reimbursement and insurance documentation.
What categories of medical receipts do you have examples for?
We currently have examples across four categories: Doctor Visit (general checkup and sick visit with copay), Specialist (specialist office visit and consultation with higher specialist copay), Urgent Care (urgent care visit and emergency room with deductible and coinsurance), and Lab & Imaging (blood test panel and chest X-ray with radiologist read fee).
Do these example receipts include the CheckoutReceipt watermark?
Yes. All examples shown here were generated using the free tier, which includes a subtle watermark. Premium users can generate receipts without the watermark. See our pricing page for details.
Will you add more medical receipt examples?
Yes. We plan to add examples including dental visit receipts, mental health therapy session receipts, physical therapy receipts, and prescription receipt formats as additional examples across all four service categories.
